Below is a list of forms to help you in your visit to Tallahassee Orthopedic Clinic. Please note that every TOC patient must fill out the TOC Patient Information Form.
Patient Information Forms
Please proceed to the Tallahassee Orthopedic Clinic Patient Portal only if one of our staff members has prompted you to do so. If a TOC staff member did not direct you to complete information contained in the TOC Patient Portal, you will be asked to complete paperwork when arriving for your visit.
TOC, in its effort to continue to provide high quality patient care, is transitioning to a new computer system. The Federal Government is requiring us to implement these changes under the PPACA (Patient Protection and Affordable Care Act). The Physicians and Staff of the Tallahassee Orthopedic Clinic thank you for your patience during the implementation of these new mandates and changes.
